The Dr. Mario game released in 1990 by Nintendo was a Tetris-like puzzle game that featured Mario as a physician armed and ready to combat three strains of virus. The viruses are colored red, blue, and yellow and need to be destroyed by aligning them with vitamin capsules which had one or two combinations of any of the three colors. The viruses are contained at various locations inside a medicine bottle, and Dr. Mario throws the capsules down the bottle one at a time. As the capsules sink down the bottle slowly, the player must rotate and align them with the virus of the same color. Successfully linking three capsule colors with a same colored virus will kill the deadly strain and bring the player closer to victory. Linking four capsule colors will also destroy the individual capsules and free up some room inside the bottle. The level is completed when all viruses are eliminated, and the player loses the game if the capsules pile up on each other until Mario can no longer fit in any new capsules inside the bottle.

At the start of every game, the options can be toggled to change the virus level, speed, and music type. Increasing the virus level means a much harder gameplay with more of the infectious organisms populating the field at a time. Increasing the speed option determines the rate in which the capsules descend, and changing the music type varies the game's audio.

A two-player versus mode is also available where two persons go head to head and try to be the fastest one to clear the viruses. The screen is split with each player having his own medicine bottle full of viruses to contend with. The virus level, speed, and music type can also be adjusted at the beginning for each individual player in order to compensate for possible skill differences between the two combatants. The first one to destroy all viruses wins the game. This adds more variety to Dr. Mario and ensures that other players are not left out.
